'Fail' means to be unsuccessful in achieving a goal or to not meet required standards. It can describe exam results ('fail an exam'), equipment breaking down ('the brakes failed'), or not doing something ('fail to understand'). 'Without fail' means certainly or reliably. 'Fail' is increasingly used as a noun in informal English ('that was an epic fail').
